This is something I found in my work confluence space. It was dated: Sep 11, 2019. I don’t remember why I put this in there. All I know is that it good information
I think of all the sections, project failure is the one to watch out for. As a government worker, the ones in bold are the big ones.

What is project?
Temporary in Nature, endeavor to create a unique product, capability or result.
Project has start/end date, expected output

Project Failure
Lack of Leadership, no one in charge or everyone

Lack of Stakeholder buy-in
Stakeholder is anyone who is affected by the project. Includes those who may stop it.
Poor scope definition
Project strays from its original goals
Unclear outcomes/expectations
Changing standards
Insufficient or mismanaged resources (budget/labor)
Poorly defined or unrealistic timeline
The project plan lacks structure and detail
The project is not tracked against the plan
Inadequate or non-existent testing with end user
Project team is not communicating
Political infighting.
